This could be either a bacteria or parasitic infection, obviously it's very difficult for me to say exactly what it could be. For reliable diagnosis, you would have to take a smear and have it tested. In the meantime, you could try a parasitic/bacterial medication and see if that works
Remember that for medication to work successfully, your water conditions must be right. If you have poor water conditions than medication doesn't normally work very well
