Do a large water change to remove the medication from the tank if that is what you used about 2 75% water changes in the same day. Siphon the gravel to remove the ich "eggs".

Do not mix the heat and salt treatment with medication treatment.

Raise the temperature no more than 1 degree an hour until you reach 87F. Add 3 tablespoons of salt per 5 gallons. Add the total dissolved amount over a 48 hour period.

The fish will get more more white spots over the coarse of 3-4 days before it gets better.

Yes it very well should. It looks like Ich to me on its head. Increase temperature slowly over the course of a couple days. When you get to 87-88 degree leave it there and watch your fish closely. You can always get back to someone on here when the ich has cleared up and they can guide you on your next steps..

I don't just add all the salt into the tank straight away do i how should I spread it out through the 48 hours

Calculate how much salt is needed for the total aquarium size, then calculate how long you will be awake to know how much salt is added per hour over a 48 hour period.

One tablespoon every 1hr and 7.5 minutes or 6 tablespoons every 6 hrs and 45 minutes. Or some combination thereof, based on your 18 tbs /48 hrs.

Take some tank water out in a container, mix and dissolve the salt, then put it back into the tank.
