how long has the fish been like this for? it's difficult to diagnose what there is, I'm leaning towards ulcer disease, or some kind of bacterial infection that has got out of control. The fish is in a very bad condition, you must take it to a vet, I don't think you're going to be able to treat the fish on your own. however, you could try a suitable antibiotic, having said that, I think you will have to get it from the vet. You must act now. Also, have you tested the water for ammonia, nitrite, nitrate and pH? There is a possibility that this is related to pour water conditions. How often do you carry out water changes?

Unfortunately, it could be too late to your fish, it looks extremely bad, it must have been going on for quite a long time, you should have taken action sooner

i do a water change every 10 days usally. about 30% of my tank wich is 29 gal

Your fish looks bad so prepare yourself for the worse. I would add an anti-bacterial or something to treat wounds, also add some salt (1 table spoon for 10g- some say 5g but that always looks loads to me)

You need to test your water for ammonia, nitrite and nitrate. Best buy your own testers because you really should test the water regularly. Can you post the results

By the way you will need a bigger tank, 55g is the minimum for one oscar
