test the water for ammonia and nitrite. often when Fish hang around the water inlet pipe it can mean that there are unnaturally high toxins in the water. It could also mean that there is not enough oxygen going into the water as well.

As for the hole in the head, please provide some photographs

Looks like a mild case of it to me.
It would be good to test your water for ammonia, nitrite and nitrate at least and tell us, since the looks alone won't really tell if it's fine for your Oscar.
Also, what are you feeding him usually? Water conditions or diet can be possible reasons for HITH.
If I were a guy, I\'d have probably needed only 30% of the amount of words to say this

Yes, unfortunately looks a little bit like a hole in the head to me. If you can, get the liquid kits, not the strips. Is your OSCAR still hanging around the outlets and is it breathing heavily? Make sure you get yourself an ammonia and nitrite test, wouldn't hurt to get nitrate one as well. Might cost you a few bob but it's an absolute necessity, without it we will find it very difficult to help you. Until you get your test kits, carry out a 25% water change every day making sure you use a water conditioner
