What are you using to fill the tank back up ? A bucket ? Water hose ?

Test tube should be shaken for 1 minute. Bottle number 2 really hard like your life depended on it for 1 minete (even if the instructions say 30 seconds).

Using hot water to bring the temperature close to your tank water will also cloud the water due the minerals from the hot water heater.

Have you tested your tap water? Let a cup of water sit overnight and test that, that will rule out nitrates in tap water.

I think you're just not quite through the cycle yet, and time should get you there.
