Because of his colouring I don't think you can appreciate just how bad this condition is got. If he was a red or Tiger Oscar then it would look 10 times worse. It will be interesting to see just how much scarring the fish is left with if you can turn this around for the better. Like Paul said, I often it can be as simple as keeping the water in really good condition that does the business. The fact that the fish is still eating is a really good sign. When they stop eating, that's when you've really got to worry..

I'm not sure if there is an issue on its anal fin or if that is just varying coloration. What do you guys think?

considering that there are no more open wounds, you are on the go...
just give him varied foods and it will heal nicely.. i can only imagine how
unique it will look when become healthy again with those scarface...
he will be the hoodlum of the oscar fish world LOL!

that mark at the anal fin is also an erosion of the outer membrane...
nothing to worry, it is also no longer an open wound

Mike, what's the trick to get the epsom salt water t soak into your pellets. I am trying to do this but they don't seem to soak up the mixture.

Pellets arent the best to absorb anything, whether it be vitamin or ES. Freeze dried feed, krill in particular works better. For pellets, I found just setting them in a spoon full of ES water for about 5 mins is about as good as you can get for saturation.

I am using Ken's fish 7.5mm pellets. It wasn't an too much of an issue to have these pellets absorb a saline solution though it was more of an issue to not over soak them to a point where they fall apart or sink to quick and go uneaten. Something like a small pill bottle may be useful to soak a feedings worth at a time. Next order with Kens I will probably try some krill and earthworm pellets, mayby a few others too.

I currently have Mike (the fish is Mike, not me) on Kens Metro medicated pellets. After the Epson salts, he was fed normally for about a week then went to a round of medicated pellets to hopefully clear the HITH. After this round I will probably move him back to normal feeding for a month or two then so another round of Epson salts and/or medicated pellets depending on progress.

His fins are looking better too. They are sticking up and displayed out more as opposed to being floppy like a sick/depressed fish.

Some updated pictures. He has went through a round of epson salts and a round of metro medicated food. He has shown some improvement but still has a ways to go to get back to as close to normal as possible.
