Disease Questionnaire for the emergency section

I have noticed we have been alot more traffic on this site and alot of new people are having fish emergency and it never fails the first response is one of the regulars asking for all the vitals so i though maybe we could impliment a disease questionnaire to help streamline the help, they do this over on the Simply Discus and it seems to work really well and its easy to just tell a new person to follow the link then copy and paste the questionnaire into there response.

Heres the questionnaire, ive changed it to suit our needs. We could put it as the top sticky in the emergency section, what do you all think??

Problem

1. Please explain the problems with your fish/when and how they started??




2. Symptoms (i.e. not eating, clamped fins, flashing, darting, clamped gills, white/yellow/green poop, hiding, headstanding or tailstanding, rotting or fungus, blisters/ white zits on fish, bloated, cloudy eyes, wounds)??




3. What medications/ treatments that you have already tried and results. Include dosages and duration of treatment??





Tank/Water

4. Tank size and age, filtration type and size (gph/lph)??



5. number and size of fish in the tank, what do you feed, how much, & how often??



6. Water change regime and conditioner used??



7. Parameters and water source;

- testing method (strips or liquid)_____

- temp _____

- ph _____

- ammonia reading ____

- nitrite reading ____

- nitrate reading ____

- municipal water or well water ____



8. Any new fish/plants added recently??

this is a good idea - there are a bunch of threads up already about how you need to provide paramters, etc., but this does seem to streamline it.

About question 5:

5. number and size of fish in the tank, what do you feed??

I would suggest we also include asking how much/how often they feed. This will help in establishing whether it's poor water conditions in the absence of a test kit, and might help diagnosing constipation.

Good idea, I will have a look at this and put it in the sticky, might even develop it for the main website as well. Thanks guys

Love this idea. Also will help us since sometimes we may forget to ask certain questions.

It works greats to streamline the process and creates consistcy for everyone helping has the info need, the biggest hurdle is getting all the regular members on board and refer them to the questionnaire, over on simply discus no one will answer a question till its filled out they just keep referring them to it

Problem

Like 6 months ago my fish started swimming sideways and had those white spots somewhere. And now there is holes. But I am really sure its not HITH.



2. Symptoms (i.e. not eating, clamped fins, flashing, darting, clamped gills, white/yellow/green poop, hiding, headstanding or tailstanding, rotting or fungus, blisters/ white zits on fish, bloated, cloudy eyes, wounds)??

Do I really have to look at the poop ? I cant find any. The holes in the head really are not that bad , so I dont think thats the problem.



3. What medications/ treatments that you have already tried and results. Include dosages and duration of treatment??

Everything the people at PetSmart told me to buy. I put it all in at the same time just to make sure I treated everything possible. The water is now purple....is that bad ?





Tank/Water

4. Tank size and age, filtration type and size (gph/lph)??

It's a pretty big tank. Like 45 Gallons. And it has one of those filters that hangs on the back...you know...water goes in one end and comes out the other.


5. number and size of fish in the tank, what do you feed, how much, & how often??

I have 7 Oscars , 2 GT's , some JD's and 3 Plecos
I feed them any time they seem hungry.


6. Water change regime and conditioner used??

My parents told me to only change 5% of the water once a month...makes sense to me. I personally use Pantene conditioner on my hair...but seriously..what does my hair to do with this ?

- ph __whats that ?___

- ammonia reading _I dont smell any___

- nitrite reading __huh ?__

- nitrate reading _its only around 60ppm accroding to my test strip___

- municipal water or well water ____

I went out and bought 7 goldfish to go into that tank. They will either do as feeder fish or fill up my tank since its a little on the empty side...

Oh, Sandra, please, please, take that description to other sites and give them the run around!

But you make a good point, sometimes, even the right questions don't get good answers. I'm tempted to say a quick explanation of how to answer might help, but making it long to read won't do us any favours here.

Maybe the sticky should be two posts long. One with just the questions, for easy copy-paste, and a second post showing a good example. But I don't know, I'm still worried about making TLDR problems.

I guess that this will at least make them say this first, and then they can be asked for more details afterwards. It still streamlines things - they answer with this sort of thing :unsure: and then we ask away, in stead of us all asking away, and then repeat.

If someone truly comes back with those kinda answers im just walking away from the thread cause not even duct tape can fix that kinda stupid!!
