You can use heat treatment in two ways. Using a temperature of between 82°F and 85°F, you can speed up the life cycle of the parasite. To completely kill the parasite out right you need to raise the water slowly to 86°F. As for salt, you don't want any more than six tablespoons per 5 gallons of water. Heat and salt treatments should be continued for 10 days. Keep treating with salt for at least three days after any visible spots have disappeared.

I would do this a bit different than Penn has suggested...

Temp to 86F.
No salt
Lots of gravel vacs
Keep temp up for 10 days AFTER the last spots are gone.

Make sure you have LOTS of airiation in the tank as higher temps deplete oxygen...

I would actually forget salt for the time being and just use heat treatment, this normally clears it up. 86°F for 10 days.
