I have had great success with my hardcore method of force feeding a mixture of food and meds into the fish with some bad cases and those fish pulled through. The non evasive approach of treating the water has seem to be one of the least effective I have tried. While it is not a fun thing for you or the fish, getting the meds inside seems to be the best approach. My mix usually will include a couple peas, Boyds vitamin, Epsom salt ground up pellets, a little bloodworm and the meds. I have used this method to successfully treat fish that stopped eating, had internal parasite and duck lips in cichlids. I need to get those meds in the fish and I might also do some metro to help with internal issues.

This hands on approach is aggressive treatment but this has been going on for too many months and I go on vacation very soon and the fish will have no special care for 13 days. If I left with it still sick it would likely be so bad when I got back I would have to put it down for sure. Tonight I begin with the food/medicine mixture.

Do you force feed the mixture into the fish?

yeah, you have to use a syringe with an airline tubing long enough to get it slightly down the throat. It takes some practice so that the tubing does not come out of the gills. I wouldn't recommend doing this for fish less than 6" in total length.

Here is my set up, a baby medicine syringe with soft silicone tubing attached:

This was a sick fish I treated for the start of duck lips, you can see mouth stuck open, and white stringy poo.
This is the regani I currently have, all better now.

What mixture of garlic guard and vitachem do you use? Do you dilute them with distilled water?

Dont have an exact answer on the wet stuff in the mix, but no water is added. Basically the meds are measured out and the instructions on the seachem products (kanaplex & focus) is followed. Then I grind up some pellet or flake food and mix into a slurry of sorts with thawed bloodworm, Boyds and garlic guard for moisture. Its been a bit of trail and error over the years but the fish I have force fed the meds have pulled through including my porkchop who got sick at an early stage in her life.

A lot depends on the fish too. If the fish is really thin I will do a high calorie food product like my Hikari Sinking carnivore (for my bichirs) to get weight on it. In the case of the Oscar who is eating and doing fine that way, I will shoot for just a more balanced food and will likely use NLS. I add a couple drops of vitamins and a tiny bit of garlic guard, along with the moisture of bloodworms or brine shrimp to get the consistency Im looking for.

Any way you could video the process? I would love to see it.

Ive never shot any video or posted any yet. Would be interesting...

I did do a write up on my local forum: www.wafishbox.com/forum/viewtopic.php?f=21&t=9803&hilit=duck+lips
Likely wont be able to see pictures without a log in but you get a pretty decent diary of events.

The fun part would be trying to video the process of holding a large fish in a wet towel and trying to get the food/med mixture into it.

Just an update.

Whatever this mess is it has not respond to any anti bacterial or antibiotics including gram negative and positive treatments. Was talking with a few others and am leaning toward a possible parasite issue, likely velvet. This makes sense since it seemed the SeaChem Paragard caused it to calm down a bit. It does not look like typical velvet and has been very slow growing since I thought I was dealing with a fungus or bacterial issue and had lowered temps which also slowed down the growth rate of the parasite. I am going to raise the temp like you do with Ich and use a combo of salt and Tetra Parasite guard which contains Acriflavin that is used to treat this. I am using this product because I always have it in my fish medicine cabinet. I did order the coppersafe which I will be able to treat the water with while I am away on vacation.

Glad to have a new game plan to try out. Thanks Lammergeier for the suggestion!

After all day in the dark I check on the little Chunk and she scraped her head. I pulled out my carbon did a water change and then added the Parasite Guard and started to add salt. I am slowly increasing the temp and salt over the next couple of hours.
